Puppy Adoption Application

 

These puppies are being sold with a limited registration which can be changed to full registration by following the terms of the warranty This is to protect against breeding without obtaining the health clearances. Dogs with limited registrations can participate in all events. The limited registration only prevents puppies produced by this dog from being registered by the AKC. We will pay for any fees for changing the registration to full after the dog has passed hip, elbow, eye, heart, and thyroid testing.

We must receive a $200 deposit and a completed application in order to reserve a puppy for you. By submitting this form and deposit it is agreed that you have read our warranty and agree to it's terms in full. Cancellations before the puppies 3 week birthday will be refunded in full. Cancellations after that date will be forfeited. Puppies are to be picked up at our location unless other arrangements have been made with us. We can ship but prefer not to. All shipping costs are paid by the buyer Please print this form and send it with a check for the deposit after an interview. Preference given to working homes.
